What causes a keyhole in a target, ammo or firearm?

Hey Snke Dr., the biggest culprit that has caused this problem for me is a dirty barrel. If attention isn't paid to lead and/or copper buildup in the barrel, you're going to have problems! I believe the excessive build up of these materials in the barrel will cause interference with the rifling and proper bullet spin. It's a very serious contender for the title of Keyhole King!!
